The Day Program

The Day Program operates Monday through Friday from 9am – 4pm, providing support for individuals with developmental disabilities age 18+ with a 3:1 or 1:1 participant to staff ratio. Please note there are limited spots for 1:1 support.

The 3:1 participant to staff ratio is for participants that can travel safely in small supervised groups throughout the community, who are generally independent in the washroom, and who do not have running away behaviours.

Full-Time Participant

Join us every day, Monday through Friday, from 9am-4pm.

Part-Time Participant

Join us by choosing the day or days that work best for you.

Occasional Participant

Join us for PD days, March Break or Summer Break.

In order to recognize their individual needs and abilities, all applicants will be evaluated during a meet and greet interview to ensure Hazelwood House can provide the support and care that is required.

Application Process:

1. Complete and submit the online application form.

2. You will then be contacted to further discuss the application.

3. If the day program is an appropriate fit for the applicant, the applicant (and their parents/legal guardians) will be given a date and time for a meet and greet interview with the program coordinators. During this time we will further discuss the support for the applicant, along with their goals, specific interests, and any possible challenges which may occur. Also enrollment fees will be discussed.

4. Within a week of the Meet and Greet, applicants will be contacted to be informed of their eligibility for the program.

5. Once offered a spot in the Day Program, applicants will receive a service contract and a start date will be confirmed.

Our programming is developed around the interests and needs of registered participants, providing both a socially and community active day program that has participants looking forward to their day!

Add-On Option

Hazelwood House offers Morning and Aftercare for participants enrolled with 3:1 support. Morning and Aftercare is offered at an additional charge of $25/hour.

Morning Care 8:00am-9:00am
After Care 4:00pm-5:00pm
